What changes when a BLS agency becomes ALS?

There is no single national equipment list that automatically converts a BLS service into an ALS service. State EMS agencies regulate EMS within their borders, and the National EMS Scope of Practice Model is guidance unless adopted by a state. Before purchasing equipment solely to satisfy a regulation, confirm the current standard with your state or regional EMS authority and medical director.

Confirm the ALS service level

Determine whether you are adding 911 ALS, AEMT-level care, paramedic ALS, interfacility ALS or another state-defined service level.

Can a BLS ambulance service become an ALS service?

Yes, but buying ALS equipment alone does not change an agency's service level. The agency generally must meet the applicable state or regional requirements for licensing or authorization, staffing, medical direction, medications, protocols, equipment and operations.

Do we need paramedics to operate an ALS ambulance?

Staffing requirements vary by state and by the level of ALS service. Some systems distinguish AEMT, intermediate or paramedic levels differently. Confirm the minimum crew configuration with your state or regional EMS authority.

Does an ALS ambulance need a cardiac monitor?

Advanced cardiac monitoring and defibrillation are common ALS capabilities, but the exact required features should be verified against your state's current equipment standard. Agencies often compare manual defibrillation, pacing, synchronized cardioversion, 12-lead ECG, SpO₂, NIBP and capnography configurations.

Is a GlideScope or video laryngoscope required for ALS?

Not universally. Some systems specify direct and/or video laryngoscopy while others leave device selection to protocols and medical direction. Verify the airway standard that applies to your agency before treating a particular device as mandatory.

Is an infusion pump required on an ALS ambulance?

Is a LUCAS 3 required for ALS?

No universal national rule makes a LUCAS 3 a requirement simply because an ambulance operates at the ALS level. Mechanical CPR is typically an agency, medical-direction, protocol and budget decision unless a local standard says otherwise.

Do we need a ventilator to upgrade from BLS to ALS?

Not necessarily. Ventilator requirements are highly dependent on the intended service level, transport mission and local rules. ALS interfacility and critical-care transport programs may have different respiratory-equipment needs than standard 911 ALS.

Do we need a new stretcher or powered cot to go ALS?

Usually not solely because the service level changes. Your existing cot must still meet applicable vehicle, patient-safety and agency requirements. Many departments evaluate powered cots and loading systems during an ALS conversion because they are already upgrading the ambulance and equipment package.

Do we need a new ambulance to become ALS?

Not automatically. An existing ambulance may be usable if it meets the ALS vehicle, equipment, space, electrical, storage and inspection standards that apply in your state or region. Confirm requirements before planning a conversion around an existing unit.

How much does a BLS-to-ALS equipment conversion cost?
